Number Porting Authorization
This form authorizes BaronTEL to transfer your existing phone numbers from your current provider to our service. Required for all number transfers. Must match your current provider's records. Fillable PDF format. Requires physical signature.
911 Service Acknowledgement
This form confirms your understanding of how VoIP 911 services work and the important differences from traditional landline 911 services. Required for all new services. Confirms your service location. Fillable PDF format. Requires physical signature.
Form Submission Steps
1. Download Both Forms

2. Complete All Fields
Fill out all required information on your computer. Make sure your information exactly matches your current provider's records.
3. Print and Sign
Print the forms and sign where indicated with a physical signature. Digital signatures are not accepted.
Submit Your Completed Forms
You have two options to return your completed forms: Email — scan and send to forms@barontel.com, or Fax — send to 647.693.9492.
Important: Both forms must be submitted before we can begin the number porting process. The number transfer typically takes 2-3 business days after form submission, but can sometimes take longer depending on your current provider.
